Treating onychomycosis topically in the second decade of new millenium

Advent of novel topical modalities for onychomycosis treatment may once again meet with late admissions and low compliance of the patients, yet impeding clinical efficacy of established antifungal treatments. Being consistent proponents of selective treatment decisions based on clinical evaluation and stratification of severity in onychomycosis, we advise the dermatological community to adhere to clinical solutions, supported by substantial evidence. Among such, administration of 1% naftifine in topical or combination regimen appears safe and effective in moderate-to-severe onychomycosis.
